To start the process, please email VasectomyReversal@mayo.edu to request a questionnaire. After you return the questionnaire, you'll be sent links to videos that go into more detail about vasectomy reversals.
You will also receive an email, typically within one to two business days, which will provide your estimated success rate based on your responses on the questionnaire.
Once you have received the email and watched the videos, you may decide if you would like to schedule surgery or an appointment for further information.
