Costs and insurance

Costs cannot always be determined in advance and will vary based on age, gender, personal and family history, symptoms, and high-risk factors.

Additional or varied testing, including colonoscopies and biopsies, may be ordered by the physician as clinically indicated and may impact the length of your stay and costs of your exam.

We offer a variety of payment options including corporate billing, insurance and self-pay. Insurance coverage for executive health services depends on your insurance carrier and plan type. Insurance carriers will process charges under the appropriate portion of your policy.

Mayo Clinic is contracted with many regional and national health insurance plans. If you have questions, contact your benefits representative or insurance carrier or both to verify the preventive health benefits in your insurance plan.

Billing is managed according to Mayo Clinic's standard insurance coverage and patient responsibility policies in Arizona, Florida and Minnesota. Some executives are corporate-sponsored, meaning that program participation is an executive benefit. Mayo Clinic works directly with patients and companies on billing and direct-billing arrangements and filing insurance claims.

Cancellation policy

Cancellation fees will be charged to patients who cancel or reschedule within 30 days of a scheduled on-site Executive Health medical exam or who fail to attend this confirmed appointment.

"The physicians at the Executive Health Program took the time to empower me to take charge of my own healthcare, which means I can live my life enjoying my family, friends and hobbies."

— Marsha Carlson
