An Executive Health Program evaluation can be scheduled at Mayo Clinic's campuses in Scottsdale, Arizona; Jacksonville, Florida; Rochester, Minnesota; or at Mayo Clinic Healthcare's London clinic in the United Kingdom.

When scheduling your appointment, we'll ask you some questions about your general health in order to help pre-schedule tests.
